PROFESSIONAL POLICIESFEES: Fees are set on a sliding scale basis using gross income and the size of the family unit as a barometer. Should income change, we will adjust the fee accordingly. We want to offer affordable fees, but without reasonable reimbursement we cannot stay open. Our fees range from $30 per hour to $100 per hour.
PAYMENT FOR SERVICE: Patients are expected to pay for services at the time they are rendered. Please notify your therapist if a problem arises during the course of therapy regarding your ability to make timely payments. Checks should be made out to Coldwater Counseling Center.
INSURANCE REIMBURSEMENT: Patients who carry insurance should remember that professional services are rendered and charged to you and not the insurance company. We will provide you with an insurance copy of the receipt, which you can submit to your insurance company for reimbursement.
CANCELLATION: Since the scheduling of an appointment involves the reservation of time specifically for you, a minimum of 24 hours notice is required for rescheduling or cancellation of an appointment. The full assigned fee will be charged for sessions missed without such notification.
CONFIDENTIALITY: All information disclosed within sessions is confidential and may not be revealed to anyone without your written permission except where disclosure is required by law. Disclosure may be required under the following circumstances: Where there is reasonable suspicion of child or elder abuse; where there is reasonable suspicion that the client presents a danger of violence to others or where the client is likely to harm him or herself unless protective measures are taken. Disclosure may also be required pursuant to a legal proceeding.
EMERGENCY PROCEDURE: If you need to contact your therapist between sessions, you can leave a message on our Voice Mail Box and your call will be returned. Please discuss with your therapist what to do should you feel an emergency situation might arise.
